2 of these:

http://www.amazon.com/Hosa-Cable-GPR.../dp/B000068O3S

Plug 'em into your mixer 1/4 outs

1 of these:
http://www.amazon.com/RCA-Stacking-P...=stackable+RCA

Plug the stacking end into the 1/4 to RCA's in your mixer outs, and the loose ends into your monitors

1 of these:

http://www.amazon.com/BestDealUSA-3-...ds=rca+to+jack

Plug it into the stackable end of the RCA, and the jack end into your laptop.

I believe this is what you are looking for :

http://www.amazon.co.uk/gp/product/B...TF8&me=&seller

I then run a single jack to jack cable from this into the PC or my Audio Recorder.

I have one of these myself for doing exactly what you want to do.
